Where Pharma Can Advertise in AI Answers: The 2027 Landscape
Last updated November 3, 2026
The question we hear most from brand teams right now is simple: where can we actually advertise in AI answers? In the past few weeks Google started testing prescription drug ads in AI Mode and ChatGPT Ads reached 89 countries. Meanwhile OpenEvidence, the clinician AI tool, is reportedly on track for up to $500 million in 2026 ad revenue.
For most brands, the bigger near-term risk is the AI answer that sits next to the ad and what it says about their product.
Ad access is uneven. A few platforms are open to pharma today, often with limits, while others have no pharma path yet. Here’s how it looks as of early November 2026, and what to do about it in your 2027 plan.
Where each platform stands
| Platform | Audience | Ad format | Rx eligibility today | What to do now |
|---|---|---|---|---|
| Google AI Mode and AI Overviews | Patients, caregivers | Standard text and Shopping ads in “Sponsored” sections | US test for certain pharma products, incl. Rx; certification required. Rx eligibility for new follow-up (multiturn) ads not stated | Prepare and test with MLR guardrails |
| OpenEvidence | HCPs (verified, US) | Pharma ads alongside clinical answers | Open to pharma; core business model | Evaluate with independent measurement |
| Doximity Ask | HCPs (US) | Commercial AI Search offering for pharma | Open to pharma; first programs live | Evaluate; early-stage product |
| Microsoft Bing / Copilot | Patients, caregivers | Search ads eligible in Copilot via AI Max; sponsored results sighted inside Bing AI Overviews | Rx search ads allowed in the US under Microsoft’s healthcare policy; in-answer pharma not confirmed | Check where existing Bing campaigns serve |
| ChatGPT | Patients, caregivers (Free and Go plans) | Ad cards below answers | Rx not on the approvable list; some health categories case by case in 6 markets | Disease awareness only, if approved |
| Amazon Rufus | Shoppers | Sponsored Products and Brands prompts (US general availability, March 2026) | Rx prohibited in Sponsored ads | Relevant for OTC and consumer health brands |
| Meta AI | Consumers | No ads in Meta AI answers | No path inside AI chat | Monitor |
| Perplexity | Consumers | Ads discontinued February 2026 | No ad path | Organic visibility only |
Google AI Mode and AI Overviews
Google is the biggest near-term opportunity for patient-facing brands. In October 2026, a Google spokesperson confirmed to ADWEEK that it is testing ads for “certain pharmaceutical products,” including prescription drugs, in AI Mode and AI Overviews, in the US only. The ads run as standard text and Shopping ads, stay out of formats where AI rewrites the copy, and still require Google’s healthcare certification. Google told Search Engine Roundtable this reflects an expansion of its AI surfaces, not a policy change.
The surface itself is growing quickly. On Oct 28, 2026, Google launched multiturn ads in the US, so ads can now appear on follow-up questions (Google). Google hasn’t said whether Rx ads qualify for those follow-up placements. On Alphabet’s Q3 call the same day, Chief Business Officer Philipp Schindler said AI Mode currently has “lower monetization” than traditional Search (Search Engine Roundtable). If AI Mode ads stay priced below Search, early tests may cost less now than they will later.
There are two catches. Google Ads doesn’t yet report AI Mode placements separately. And ad density in health is still low: SE Ranking found ads on 2.64% of commercial healthcare prompts in AI Mode (Search Engine Journal).
The unresolved fair balance question
When Google first opened AI Mode to healthcare ads in June, the test excluded creatives that use pinning or text disclaimers. That ruled out most branded Rx creative, which depends on fair balance language and safety information. Adthena tracked 50,000 health-adjacent AI Mode placements in August and found zero from prescription brands.
The October Rx test is the first sign that gap is closing. But Google hasn’t published how risk information should render inside an AI answer, or whether the one-click path to Important Safety Information behaves the way it does on a standard results page. Until it does, put two questions to your MLR team:
- Do we start with reminder ads (brand name only, no indication or claims) to sidestep the problem?
- What does our ISI path look like on a screenshot of a live AI Mode ad, not a mock-up?
OpenEvidence
OpenEvidence is the clearest HCP-facing AI ad business. It is free to clinicians and funded largely by pharma advertising. On Oct 21, 2026, the Financial Times reported that the company says nearly two-thirds of US physicians use it, and a person close to the company said it is on track for $400 million to $500 million in 2026 ad revenue (FT via Ars Technica). That figure is a forecast from an unnamed source; the company hasn’t reported it. Becker’s Hospital Review has reported that ads appear on about 5% of queries.
Two things to weigh before you commit budget. Independent performance data is still thin, so test with your own measurement. And the model itself may change: in May, CEO Daniel Nadler was reported to be evaluating a shift from free, ad-supported access to paid subscriptions. Until that’s settled, fund OpenEvidence from a test budget.
Doximity
Doximity is moving into the same space, and the product is already in market. On its May 2026 earnings call, CEO Jeff Tangney said the company had launched a commercial AI Search offering aimed at pharma’s paid search budgets and closed its first deals with top 20 pharma manufacturers (MarketBeat transcript). By its fiscal Q1 2027 report (April to June 2026), Doximity had onboarded a first cohort of AI Search customers across more than two dozen programs, though it hadn’t yet recognized AI revenue. AI Search queries grew more than 25% quarter over quarter (Doximity).
The clinical product is getting stronger too. In October, Doximity and Wolters Kluwer announced that UpToDate content will appear alongside Doximity Ask answers for clinicians at health systems that use both (Wolters Kluwer). For advertisers, that raises the credibility of the surface your message sits next to. Expect pricing and format detail to firm up through 2027.
Microsoft Bing and Copilot
Microsoft already allows prescription drug search ads in the US, and its AI Max for Search campaigns, announced in April 2026, can deliver ads on Copilot Search and Copilot Answers. Ads have also started appearing inside the answers themselves. Search Engine Watch reported Bing testing sponsored results inside its Copilot results this summer, then expanding them on Sep 1. On Oct 30, Search Engine Roundtable reported a screenshot of a sponsored result inside a Bing AI Overview (Bing’s new label for Copilot answers), which it could not reproduce.
Microsoft hasn’t announced in-answer ads, and none of these sightings involved pharma. For brands already on Bing, it’s worth checking where those campaigns are eligible to serve and whether Rx creative can appear in Copilot surfaces.
ChatGPT
ChatGPT has scale. Ads now run in 89 countries, and more than 100,000 advertisers have run campaigns since launch, per OpenAI (Oct 21, 2026). But branded Rx isn’t on the menu. OpenAI’s ad policies allow some health advertisers case by case, including disease awareness campaigns, in the US and, since August, the UK, Mexico, Brazil, Japan and South Korea. Prescription drugs aren’t on that list.
ChatGPT already sells ads around health questions, though not for Rx. In April, OpenAI stopped blocking medical-advice contexts from ads by default, while still keeping ads off personal health conversations. SE Ranking found ads on 28.69% of commercial healthcare prompts in ChatGPT, more than ten times the rate it measured in Google’s AI Mode (Search Engine Journal).
For pharma, that makes ChatGPT a visibility priority first and a possible unbranded channel second. Any disease awareness campaign still has to meet FDA’s rules for help-seeking communications: no product name, and no path to a branded site.
Amazon, Meta, and Perplexity
Amazon’s Sponsored Products and Sponsored Brands prompts inside its Rufus shopping assistant moved to general availability in the US in March 2026 (Acadia). Amazon’s ad policies prohibit prescription drugs in those placements, so this matters for OTC and consumer health brands, not Rx.
Meta allows authorized prescription drug ads on Facebook and Instagram (Meta), but it hasn’t announced ads inside Meta AI answers. Since December 2025, Meta has used Meta AI conversations to personalize ads elsewhere on its platforms (TechCrunch). Ask your Meta team how health-related chats are treated before assuming that signal helps or hurts your targeting.
Perplexity was an early tester of sponsored follow-up questions, then confirmed in February 2026 that it is leaving advertising to protect user trust (Search Engine Land). Organic visibility is the only route there.
What to have ready, regardless of platform
The preparation is the same on every platform, and it starts with the AI answer.
- Know what AI says about your brand. Your ad will sit next to an AI answer, and people judge the two together. Audit what Google AI Mode, ChatGPT, OpenEvidence, and Doximity Ask say about your brand and competitors, then save it as a baseline. If the answer is wrong or incomplete, fix that before you buy the placement beside it.
- Agree with MLR on green and red conversation zones. With follow-up ads, your message could appear several questions into a conversation. Decide in advance where an ad belongs and where it doesn’t, for example after a question about side effects or stopping a medication. On the HCP side, remember Practice Fusion’s $145 million settlement over pharma-funded alerts inside an EHR. Regulators take commercial messages placed in clinical decision moments seriously.
- Keep HCP and patient plans separate. OpenEvidence and Doximity reach clinicians; Google, Bing, and ChatGPT mostly reach patients and caregivers. Each audience needs its own messages and budget, measured against goals built for that audience.
- Build measurement you can prove. Most AI placements aren’t reported separately yet. Set clear conversion definitions, track independently, and run holdout tests before spend scales, so you can show leadership what these channels actually deliver.
A note on moving policies
Everything here is changing month to month. Google’s Rx test has no dedicated public policy page yet, and OpenAI has changed its ad eligibility rules four times since March. Check each platform’s current terms before committing budget.
That’s why preparation belongs in the 2027 plan. Brands that have done the groundwork can act within weeks when a platform opens to them.
The bottom line
AI answers are becoming ad inventory across both HCP and patient journeys. The advantage will go to brands that already know what AI says about them and can show leadership what each placement is worth.

Frequently asked questions
Can prescription drug ads run in Google AI Mode?
Yes, in a limited US test that began in October 2026. Ads run as standard text and Shopping ads in “Sponsored” sections, and advertisers need Google’s healthcare certification.
Can pharma companies advertise prescription drugs in ChatGPT?
Not as of November 2026. OpenAI allows some health advertisers case by case, including unbranded disease awareness campaigns, but prescription drugs aren’t on the approvable list.
Which AI platforms let pharma reach HCPs?
OpenEvidence and Doximity both sell pharma advertising alongside AI-generated clinical answers for US clinicians. Both are early, so independent measurement matters.
Does Perplexity sell ads?
No. Perplexity confirmed in February 2026 that it is leaving advertising. Organic visibility is the only route.
Does Meta show ads inside Meta AI?
No ads inside Meta AI answers have been announced. Meta does use Meta AI conversations to personalize ads on its other platforms.
